FAMI allowance in France: how physiotherapists declare it
FAMI allowance for physiotherapists: where to click on amelipro, what to enter, which documents to attach. Assessed 31 December, declared the next year.
To qualify for the FAMI allowance (forfait d’aide à la modernisation et à l’informatisation, the aid for modernising and computerising the practice), a physiotherapist covered by the national agreement must declare some of their indicators themselves on amelipro (the Assurance Maladie online portal for healthcare professionals), every year, during a limited entry period that opens after the end of the financial year: for the 2025 year, it ran from 12 January to 2 March 2026. This article is a tutorial: it follows, in order, what the official documents of the Assurance Maladie (French national health insurance) describe, from logging in to keeping the summary, with direct links and the documents to have ready.
For the 2025 year, the allowance could reach €1,115 per year (€490 base tier, €100 coordination, €175 connected devices, €350 secure video transmission), and whether the indicators are met is checked on 31 December, with no pro rata; the declaration itself is made the following year. The steps draw on the ameli page on the allowance, the Cnam methodology guide (Cnam: the national health insurance fund) and the memo “Comment déclarer vos indicateurs”, all relating to the 2025 year and re-read on 14 October 2026. The documents for the 2026 year have not been published yet: screen labels and dates may vary, and each uncertain point is flagged as such.

What you need before you start
Your amelipro access: username and password, or a CPS card (the healthcare professional’s smart card).
The name, version and publisher of your practice software, and its acquisition date.
Proof of the software, if your first declaration, a change of publisher or a box that is not pre-filled requires it: an invoice, purchase order or certificate from the publisher, in your name, bearing the words “DMP compatible”.
The type of your secure health messaging. If you are a self-employed collaborator, the certificate from the practice holder.
Where applicable, the structure of your coordinated practice (MSP, ESP or ESS, CPTS, other).
Where applicable, your secure video transmission subscription and the connected device from the official list that you have.
The dates of the entry period, which have not been published for the 2026 year.
Step 1: know what is measured and what is declared
The FAMI allowance is provided for by avenant 5 (amendment 5) to the national agreement for physiotherapists. It brings together the aids previously paid (the teletransmission aid, the maintenance aid and the SCOR aid). According to the 2025 guide, whether the indicators are met is checked on 31 December of the reference year, with no pro rata at all: for the 2026 year, unless the 2026 guide changes this, the date on which indicators are assessed will be 31 December 2026 and the declaration will be made in 2027.
| Tier | Amount | Indicator | Who fills it in |
|---|---|---|---|
| Base: all five indicators must be met | €490 | Practice software compatible with the DMP (shared medical record) | you, with possible pre-filling (step 6) |
| Version of the SESAM-Vitale specifications (the national electronic claims system) | the Assurance Maladie, automatically | ||
| SCOR solution (at least one flow in the year) | the Assurance Maladie, automatically | ||
| Rate of electronic claim forms of at least 70% | the Assurance Maladie, automatically | ||
| Secure health messaging | you, with possible pre-filling (step 7) | ||
| Coordinated care | + €100 | Coordinated practice; paid if the base tier is met | you (step 8) |
| Telehealth | + €175 | Connected medical devices | you (step 9) |
| Telehealth | + €350 | Secure video transmission | you (step 9) |
Maximum total for the 2025 year: 490 + 100 + 175 + 350 = €1,115. The two telehealth indicators give rise to payment that is independent of whether the base indicators are met: they are declared even if the base tier is not met. Payment is the responsibility of the Assurance Maladie.
The three automatic indicators are not entered by you: the Assurance Maladie measures them itself.
SESAM-Vitale specifications: measured from the billing flows sent by 31 December at the latest, it assumes you are equipped with the required version (version 1.40 addendum 8 for the 2025 year) and have billed in compliance. For 2026, the guide announces that the Maintenance Addendum 8 2024 amendment will also be included, to be confirmed in the 2026 guide.
SCOR: at least one SCOR flow sent in the reference year.
Rate of electronic claim forms: at least 70%, with certain procedures excluded from the calculation.
For SCOR and the rate of electronic claim forms, the guide states that you can check whether the target has been met on amelipro, in the payment details, when these are available for the year.
Step 2: spot when the entry period opens
The declaration is made every year, after the assessment date of 31 December, during a limited entry period: it is not a step to complete before 31 December. For the 2025 year, entry was open from 12 January to 2 March 2026 at midnight, and the guide provides for payment of the aid in the second quarter of 2026. The 2025 guide presented these entry dates as “provisional”.
For the 2026 year, as at 14 October 2026, the ameli page offers only the 2025 edition documents: the entry dates have not been published. The dates of the campaign held in early 2026 are a benchmark, not a deadline. Keep an eye on the ameli page on the allowance, where the guide, the memo and the form for the 2025 year currently appear.
Step 3: log in to amelipro
Open amelipro, the “Accéder à amelipro” link on the ameli page for physiotherapists. The official memo gives two ways of logging in: with your username and password, or with a CPS card.
Step 4: open the “Ma convention” online service
On the amelipro home page, under the “Activités” heading, click the “Convention - ROSP” link. The dashboard of the “Ma convention” online service is displayed: it allows you to enter the declarative indicators and to view the payment details. The memo’s model shows a “Ma déclaration” block, a counter of indicators already declared and a “Déclarer mes indicateurs” button.
The label may vary: the official paper form refers to the same path as the “Ma convention” heading, accessible via the “Indicateurs Convention médicale (ROSP)” tab of the “Activités” heading. If the link described above does not appear identically on your screen, look for these two labels.
Step 5: read the declaration screen
The official memo presents a model entry screen, “Déclarer mes indicateurs”, which lists six lines. Note: the memo’s screenshots come from earlier years and are small, some hard to read. The table below follows this model, cross-checked against the screen extracts in the guide; the actual screen for your year may show different labels.
| Line | Model label | On-screen control | What you do |
|---|---|---|---|
| 1 | Practice software | “Renseigner” button | check or enter your software (step 6) |
| 2 | DMP-compatible software | “Renseigner” button | check or enter DMP compatibility (step 6) |
| 3 | Type of secure health messaging | drop-down list | select an answer (step 7) |
| 4 | Coordinated care | drop-down list | select an answer (step 8) |
| 5 | Video transmission equipment | Yes or No choice | answer (step 9) |
| 6 | Connected medical device equipment | Yes or No choice | answer (step 9) |
According to the memo, if all the indicators are pre-filled and you change nothing, there is no need to submit the declaration: the submit button stays greyed out. This statement is hard to read on the model screen; check it on your own screen.
Step 6: declare the practice software compatible with the DMP
This base indicator is declarative, with pre-filling. If, during the year, you carried out at least one action on a DMP (adding to it, consulting it, managing administrative data, searching for a DMP) with DMP-compatible software, the DMP-compatible software line is filled in automatically from the traces in the DMP information system, and a first load takes place before entry opens. In that case, check the pre-filled information, correct it if necessary and provide the documents that support a correction. Use of the “Web DMP” does not validate the indicator. The guide also states, in its description of the indicator, that at least one flow must have been carried out in the year, whatever its type; yet it describes a manual declaration where there is no interaction with a DMP. The two passages cannot be clearly reconciled: if you are in this situation, ask your fund.
With no interaction with a DMP at all in the year, you declare the details of your equipment yourself from a drop-down list, available directly in the entry section, and you must then send the supporting documents to your fund of affiliation. The guide mentions drop-down lists, the model screen a “Renseigner” button: the content of the window it opens is not documented. The documents must make it possible to identify the publisher, the name of the software and the acquisition date; for DMP compatibility, the proof must be in your name, bear the words “DMP compatible” and state, where applicable, the use of another publisher (connector) or the purchase of an additional pack. An invoice, purchase order or certificate from the publisher (template in annex 1 of the guide) are acceptable. When declared by hand, the indicator is validated only if the supporting documents are actually sent.
Supporting documents are required only in the first year of entry, in the event of a change of publisher, or if the box is not pre-filled. Once provided, they remain valid for the duration of the agreement, and the words “pièce justificative validée par la CPAM” (supporting document validated by the CPAM, the local health insurance office) appear under the indicator, according to the guide. The indicator is also assessed on 31 December, regardless of the acquisition date of the equipment.
Step 7: declare the secure health messaging
The indicator is met if, before 31 December of the reference year, you use a messaging service from the MSSanté trust space (the secure health messaging network), personal or organisational. For Apicrypt, only version 2 is taken into account, with the “Interopérabilité avec l’Espace de Confiance” box ticked at installation.
If the national MSSanté directory has detected a messaging service, the indicator is pre-filled and no supporting document is requested. Otherwise, select your type of messaging in the drop-down list on line 3: you thereby declare on your honour that you have it, and you undertake to keep the supporting documents available in case of inspection. Select only what you actually had before 31 December of the reference year: the supporting documents may be requested during a subsequent inspection. Special cases: organisational mailboxes allow no automatic pre-filling, and a self-employed collaborator validates the indicator with a certificate from the practice holder stating that they are making their messaging available, accompanied by a copy of the supporting documents for the practice’s equipment.
Step 8: declare coordinated care
According to the guide, this additional indicator is paid €100 if the base tier is met. In the drop-down list on line 4, you select the scheme you take part in: a multi-professional health centre (MSP), a primary care team (ESP) or specialised care team (ESS), a territorial professional health community (CPTS), or another coordinated practice (at least four multidisciplinary consultation meetings a year, a local network or care network, coordination actions for home hospitalisation (HAD) or a care home for dependent older people (EHPAD) as part of self-employed activity).
The following do not validate the indicator: a quality group, a PAERPA project, individual coordinated care following a request from a DAC, the PRADO programme. Your selection counts as a declaration on your honour; you keep the supporting documents available in case of a subsequent inspection.
Step 9: declare the telehealth equipment
Lines 5 and 6 call for a Yes or No answer on the model screen; the guide, for its part, speaks of ticking a box. No supporting document is to be sent to the fund: you declare on your honour, on amelipro, that you had compliant equipment during the year, and you undertake to keep the corresponding supporting documents available to the Assurance Maladie in case of a subsequent inspection. Answer Yes only if you actually had the equipment.
Video transmission (€350): you declare that you are equipped with a complete secure video transmission solution necessary for teleconsultation, or that you have taken out a subscription to a telehealth solution from an operator offering a complete, paid technical solution. Free consumer solutions (Skype, FaceTime, WhatsApp, Zoom) are not sufficient on their own.
Connected medical devices (€175): you declare the purchase or rental of a device from the list drawn up for 2025: spirometer, impedance meter, pedometer, dynamometer, stabilometry platform, oximeter, stethoscope, dermatoscope, blood pressure measurement, all connected. The list may change every year.
Step 10: send the supporting documents
According to the memo, you can attach the supporting documents by email to your CPAM as soon as entry opens. In the guide’s screen extract, a message invites you to “joindre les pièces justificatives requises pour cet indicateur” (attach the supporting documents required for this indicator) and an envelope icon appears next to the indicator. For its own submission, the paper form mentions the amelipro email function with the address model “Conventionmed.cpam-XX@assurance-maladie.fr” (XX standing for your fund), or sending by post to your CPAM; the text of the memo mentions only email for supporting documents, and its model screen, hard to read, seems also to mention postal sending.
Send only what is required, where applicable: proof of the software (step 6). A self-employed collaborator must, for their part, have the certificate from the practice holder (step 7). The three automatic indicators require none. The same applies to video transmission and connected devices (guide). For coordinated care, the declaration on your honour is made by selecting the item in the list (step 8).
Step 11: tick the two boxes and submit
According to the memo, to submit your declaration you must confirm that you have read the general conditions, certify that the information entered is accurate and undertake to keep the supporting documents available to the Assurance Maladie if necessary, by ticking the two boxes provided for this purpose. Then click the submit button, whose exact label is not legible on the model screen.
Entry does not have to be done in one go: the memo states that you can submit a partial declaration as many times as necessary. This flexibility does not extend the period: it ends on the stated date, at midnight.
Step 12: download and keep the summary
The memo describes two PDFs. The first summarises all the declarative and calculated indicators that were used to calculate the payment, showing whether or not each target was met. The second, “Ma dernière déclaration” in the useful documents, is the summary PDF of the previous year’s entry. According to the guide, a summary of the indicators declared on your honour is made available on amelipro at the end of entry and must be kept: it serves as proof in case of inspection.
If you are stuck
| Situation | What the official documents say |
|---|---|
| You do not have access to amelipro | As an exception, the declaration can be made on the paper form handed to your fund of affiliation, identified by your professional stamp and signed, before the end of the entry period. The form mentions sending by email via amelipro or by post to the CPAM. |
| The DMP-compatible software line is not pre-filled | Declare it by hand and send the supporting documents to your fund; without them, the indicator is not validated. |
| The secure messaging line is not pre-filled | Select your type of messaging in the list: the selection counts as a declaration on your honour, with no document to send, the supporting documents remaining to be kept available. |
| You have changed software publisher | New supporting documents are needed, otherwise the indicator is not validated. |
| An automatic indicator does not appear to be met | It is not declared: the Assurance Maladie measures it. The documents describe no procedure for challenging it; check the payment details (the guide mentions this for SCOR and for the rate of electronic claim forms) and go through the contact page below. |
| Your problem is of another kind | The page Contacter l’Assurance Maladie offers the entries “amelipro, les logiciels et votre équipement” and “Votre convention et formation professionnelle”. |
What the official sources do not specify
Neither the guide, nor the memo, nor the ameli page documents the following points as at 14 October 2026, so this tutorial does not assert them:
what happens if the entry period is missed: no catch-up procedure is described;
the content of the window opened by the “Renseigner” button, the options in the drop-down lists, the confirmation message after submission and the exact label of the submit button;
the fund’s response time, the time to validate documents and any error messages on the screen;
the file formats and sizes accepted for the documents (a line of the model screen seems to state them, but it is illegible);
the exact screen for the 2026 year, whose labels and dates have not been published. The memo’s screenshots date from earlier years: they are an illustration, not a screen guaranteed to be identical.
And for other healthcare professions?
Are you a podiatrist, orthoptist or speech therapist?
According to the Cnam 2025 methodology guides, your scheme is the same as the physiotherapist’s for the 2025 year: same indicators, same amounts, same provisional entry period, same declaration memo, and a maximum total of €1,115 (with a reservation for the speech therapist, below). This tutorial can therefore serve as a guide for you, with three precautions:
The list of connected devices for the €175 allowance is specific to your profession: see annex 2 of your guide (podiatrist, orthoptist, speech therapist).
Speech therapists: the guide shows a total of “1115 (ou 1015)” without explaining it, and avenant 21 adds an allowance of €69 per intervention in schools from the 2026 year (details in the table below).
Steps 4 to 12 describe the amelipro screens based on the physiotherapist’s documents. Nothing establishes that the screens are identical for your profession: check them against your guide.
For the other professions, what follows places each scheme according to the Cnam 2025 methodology guides and the ameli pages re-read on 15 October 2026. The figures are those of each profession’s guide, never a transposition of the physiotherapist’s. The guides all announce the same provisional entry period for the 2025 year, from 12 January to 2 March 2026.
| Profession | What the sources say | To check |
|---|---|---|
| Podiatrist | Same scheme as the physiotherapist according to the 2025 guide: five base indicators (€490), coordination (€100), video transmission (€350), connected devices (€175), maximum total of €1,115. Only the list of connected devices is specific to the profession. Ameli page of 23 August 2024, whose address goes through “exercice-professionnel”. | Ameli page · 2025 guide |
| Orthoptist | Same scheme as the physiotherapist according to the 2025 guide: same indicators, same amounts, maximum total of €1,115. Only the list of connected devices is specific to the profession. This guide bears the note “MAJ janvier 2026” (updated January 2026), the others “octobre 2025”. | Ameli page · 2025 guide |
| Speech therapist | Same scheme as the physiotherapist for the 2025 year: same indicators and same amounts. Two reservations. The guide’s table shows a total of “1115 (ou 1015)” without detailing the alternative; the circular states only that coordination (€100) is now an additional indicator and no longer part of the base tier, and the reason for the second figure is not established. And avenant 21 adds, from the 2026 year, an allowance of €69 per intervention at meetings with educational teams in schools (5 a year at most, supporting document requested, independent of the base tier), to be declared in 2027 for 2026. As at the date of re-reading, the guide for the 2026 year, which will set the total, has not been published. | Ameli page · 2025 guide · circular CIR-5/2026 · ameli news item |
| Nurse | A scheme specific to the nurses’ agreement, with rules specific to self-employed advanced practice nurses: a coordinated practice bonus of up to €1,020 in an area with a low density of doctors and €300 outside such an area, and an optional SCOR indicator for exclusive advanced practice. The amounts in the scheme can be read in the profession’s guide. Ameli page of 9 January 2024. | Ameli page · 2025 guide |
| Midwife | A different scheme: four base indicators (€490), SCOR being optional for the profession, coordination (€100), the two telehealth indicators (connected devices €175, video transmission €350), then three indicators independent of the base tier (“aller vers” outreach €300, hosting a trainee €350, intervention in a healthcare facility €1,000 or €2,000). The 2025 guide shows a maximum total of €3,765. Ameli page of 14 February 2025. | Ameli page · 2025 guide |
| Dentist | A different scheme: five base indicators (€490), including display of opening hours in the health directory, in place of SCOR, coordination (€100), no telehealth indicator, and two independent indicators (“aller vers” outreach €300, hosting a student on placement €350). The 2025 guide shows a maximum total of €1,240. The €350 amount concerns the training supervisor, not video transmission. Ameli page of 26 February 2024. | Ameli page · 2025 guide |
| Doctor | No FAMI. The forfait structure (practice structure allowance) ended on 31 December 2025, its last payment being made in 2026 for 2025. Since 1 January 2026, it has been replaced by the digital allowance (dotation numérique, Donum), whose ameli page of 15 September 2026 mentions no video transmission indicator. | Forfait structure · Donum |
| Pharmacist | A separate scheme, which is not the FAMI: teleconsultation assistance in pharmacies, with its own payment and its own conditions. Page of 29 May 2026. | Ameli page |
Is the declaration made in the same place? The ameli pages for nurses, speech therapists, orthoptists, midwives, podiatrists and dentists all state that the indicators are declared every year on amelipro, during the entry period, and refer to the same practical guide, the memo “Comment déclarer vos indicateurs” already cited above. An identical scheme of indicators says nothing about the screens: none of these pages specifies whether the amelipro screens are the same from one profession to another. The guides give the same entry point, the “Ma convention” service under the “Activités” heading (“onglet Ma convention” in the nurses’ guide), but a shared entry point tells us nothing about the content of the screens. Podiatrists, orthoptists and speech therapists can follow this tutorial as a guide by checking each screen against their guide; for nurses, midwives and dentists, whose schemes differ, it is better to start from the profession’s methodology guide (links in the table). The doctors’ ameli page refers, for the forfait structure, to a separate guide.
What Markus changes, and what it does not
Markus is not billing software. It opens no entitlement to the €490 base tier, the €100 coordination allowance or the €175 for connected devices: these three tiers depend on your practice software, your secure messaging, your coordinated practice and your equipment. Markus declares nothing on your behalf and does not guarantee payment of the allowance, which is the responsibility of the Assurance Maladie.
Only the video transmission indicator may concern Markus, and no Markus plan on its own entitles you to the €350. The Markus Consultation plan includes video appointments; the Markus Assistant plan, without the schedule, does not. Prices noted on the website’s pricing page on 27 October 2026, on the monthly plan per practitioner: Markus Assistant €28.80 including VAT per month (€24 excluding VAT), Markus Consultation €58.80 including VAT per month (€49 excluding VAT). Whether the subscription meets the definition of the indicator (a complete and secure solution) is neither guaranteed nor attested by Markus: it is up to you to judge whether it does before, where applicable, declaring it on your honour. For the allowance itself, only the official documents cited in this article are authoritative.
Frequently asked questions
Does the base tier have to be met to declare the telehealth aids?
No. The two telehealth indicators give rise to payment that is independent of whether the base indicators are met (Cnam/DDGOS methodology guide, 2025 year).
Does the declaration have to be redone every year?
Yes. The indicators are declared every year on amelipro during the entry period, for the year just ended. According to the memo, submission would not be necessary if everything is pre-filled and unchanged; this statement is hard to read on the model screen, so check on your own screen.
Which documents should be kept?
The summary PDF of the entry, together with the supporting documents corresponding to your declarations on your honour, to be presented in case of a subsequent inspection.
When is the aid paid?
For the 2025 year, the guide provides for the second quarter of 2026. The calendar for the 2026 year remains to be confirmed with the Assurance Maladie.
And for other healthcare professions?
The tutorial describes the physiotherapists’ allowance; the other professions have their own indicators and amounts. The detail, profession by profession, is in the dedicated section.
Your equipment on 31 December: points to check
31 December is the date on which the Assurance Maladie assesses your indicators: these points concern your equipment and your rates on that date, not the declaration, which is made the following year during the entry period.
Base tier, €490: your practice software is compatible with the DMP, and at least one action on a DMP was carried out in the year (otherwise, see step 6: the guide is not clear on this case).
Base tier: your equipment is up to date with the required version of the SESAM-Vitale specifications, and at least one SCOR flow has been sent.
Base tier: your rate of electronic claim forms reaches 70%.
Base tier: you have secure health messaging from the MSSanté trust space.
If you take part in a coordinated practice, you will be able to declare it if you meet the guide’s definition (amount provided for 2025: €100, paid only if the base tier is met).
If you are equipped with a complete secure video transmission solution, or subscribed to a complete, paid telehealth solution, you will be able to declare it if you meet the guide’s definition (amount provided for 2025: €350).
If you have a connected device from the list, you will be able to declare it if you meet the guide’s definition (amount provided for 2025: €175).
