Speech therapy software in France: 2026 FAMI criteria
SESAM-Vitale billing and teletransmission, DMP, SCOR: the criteria for speech therapy software and how they relate to the €490 FAMI allowance in 2026.
For the 2026 financial year, assessed on 31 December, €490 of support depends in part on your software. According to the French national health insurance (Assurance Maladie), the practice modernisation and computerisation allowance (forfait FAMI) pays this amount to speech therapists who meet five indicators, and three of them depend on what your software can do: compatibility with the shared medical record (DMP), the version of the SESAM-Vitale specifications, and SCOR, the digital transmission of supporting documents.
This guide sets out what software must do to bill and teletransmit, what the Assurance Maladie measures, and the questions to ask a vendor before you sign.
Billing and teletransmission software for speech therapists: what it must do
Practice management software for speech therapy has three basic functions: keeping the patient record, billing procedures and teletransmitting claim forms. The last two are governed by precise rules published by the Assurance Maladie.
Billing and teletransmitting with SESAM-Vitale
Teletransmitting electronic claim forms (FSE) requires a healthcare professional card (CPS) and computer equipment. The Assurance Maladie sets out the following points on its page on teletransmission for speech therapists (updated 7 January 2026):
The software must be approved by the national approval centre (CNDA) and certified by the GIE SESAM-Vitale, whatever the way it is acquired (purchase or rental).
The workstation must run on PC/SC. Since 1 January 2022, only SESAM-Vitale solutions using this standard are supported. Both the billing software and the card reader must be compatible with it.
The version of the specifications matters. The version required for the 2026 financial year is 1.40, addendum 8, which notably allows billing with the carte Vitale app.
Plan ahead for home visits. Since 1 July 2025, application reader terminals (TLA) are no longer supported by the GIE SESAM-Vitale, which points towards mobile solutions. Check what your vendor offers for billing outside the practice.
If you have a secretary, they can be equipped. A staff card (CPE) lets them read the patient's carte Vitale and create a pre-FSE.
The CPS card is free. Issued by the regional health agency, it is valid for three years and can cover up to eight practice locations. Allow five weeks on average to receive it.
SCOR, DMP, secure messaging: the allowance's other requirements
Three other requirements count towards the FAMI allowance, in addition to the version of the specifications and the teletransmission rate. The first two depend on your software, the third on your messaging set-up:
SCOR: digital transmission of supporting documents to the Assurance Maladie. At least one SCOR flow must be sent between 1 January and 31 December 2026.
Practice software compatible with the DMP (shared medical record). According to the methodology guide, the "Web DMP", meaning access to the record from a browser, does not meet this indicator: you need a compatibility component built into the practice software, or a connector.
A secure health messaging service that complies with the MSSanté trust space.
FAMI allowance 2026 for speech therapists: the five indicators worth €490
According to the Assurance Maladie, the allowance brings together the former support for teletransmission, maintenance and SCOR. It is paid once a year, as a single payment. The rules are set by the Assurance Maladie pages for speech therapists and its 2026 methodology guide (updated September 2026).

According to the methodology guide, all five indicators must be met to receive the €490. Assessment takes place on 31 December, with no pro rata: setting up during the year does not reduce the amount. Four supplements exist. The first, €100, rewards coordinated practice and is paid only if all five indicators are met. The other three are paid regardless of the five indicators: €350 for secure video transmission equipment, €175 for connected medical devices and €69 per school intervention, up to five per year. The maximum total is €1,460.
Again according to the guide, self-declared indicators are entered on amelipro from 11 January to 1 March 2027, in the "Ma convention" service, and payment for 2026 is made in the second quarter of 2027.
Two practical consequences for choosing software. First, only indicators 1 and 5 are self-declared: the other three are calculated automatically by the Assurance Maladie from your billing flows. Second, the guide states that if you change vendor, the proof of DMP compatibility must be sent again to your local health insurance office, otherwise the indicator is not validated.

Practice management, billing and teletransmission
This is the family the FAMI allowance targets directly. It covers scheduling, the patient record, billing and SESAM-Vitale teletransmission.
Orthomax by Topaze presents itself as 100% web and mobile speech therapy software for teletransmission and billing, with writing and management of assessment reports (CRBO), built-in telecare, mobile billing and data stored with an HDS-certified host. The vendor states that it is eligible for the FAMI allowance.
VEGA is presented by its vendor as management and teletransmission software for self-employed allied health professionals, including speech therapists, approved for SESAM-Vitale. The vendor states that the software meets the technical prerequisites of the FAMI allowance and that two conditions remain your responsibility: a secure billing rate of at least 70% and secure messaging.
Appointments, assessments and therapy
Other tools sit alongside the management software: online booking, administering and scoring standardised tests, therapy materials. They are not among the allowance's five indicators, but they matter day to day. Is the Web DMP enough to meet the DMP indicator?
